We have a great opportunity to work with two local, innovative and fast expanding surgeries alongside current members of staff.
We are looking for an enthusiastic Clinical Pharmacist to work with us in developing our ambitious plans of providing primary health care to our patient population. Experience of successful change management and delivering education and training is desirable.
The Clinical Pharmacist role within our surgery will dynamically help patients stay safe and well and out of hospital, as well as helping reduce the demands on general practice.
Main duties of the job
1. To offer comprehensive acute and long-term support for the practice to provide clinical tools and resource to help manage patients with long-term conditions, medication requests and acute issues
2. Management of related QOF indicators inclusive of long-term condition medications with additional involvement of implementing and using protocols and clinical guidelines, both at practice level and locally, governed by the ICB or NHS
3. To identify opportunities for clinical reviews at individual practice, PCN or CCG level where there is clinical need, working with the remit of qualifications.
4. Educating patients on their medications, including the medication dosage, any potential medication interactions and side effects
5. To enhance the awareness of alternative Clinical Services within primary care and to assist in developing long lasting relationships with practices and local NHS organisations, utilising these to improve patient care
6. Utilise any gained specialist field knowledge to offer extended support to the practice and the patients within this demographic

Both The Birches Medical Centre and Martlesham Heath Surgery are pleased to announce our connection with the new Wolsey PCN, this new collaboration is between a select number of GP practices within the local area. This venture offers a significant part of our future GP funding, as such the PCN may fund your post but you will be based with our teams.
Our patients are often happy to take responsibility for their own health, however, we offer on the day contact to a wide selection of teams when they feel help is needed. The main access for our patients is via Anima, this has made a great improvement to the access we offer our patients and we are always looking at new technology to aid our patient journey.

Job responsibilities
The remit of our Clinical Pharmacist is to be confident working with and on behalf of the Clinical Teams.
The focus and main responsibilities within the role is to deliver exceptional patient care, directing experience and knowledge to patients surrounding their medications. Highlighting and implementing management and preventative interventions where applicable, discussing and promoting Over The Counter (OTC) availabilities where appropriate.
Daily consultations will consist of (but not limited to)
7. Offering advice to patients related to their medications and medical problems
8. Monitoring patient prescription use to ensure they don't abuse medication
9. Recommending the most appropriate administration routes based on patient needs
To work within the governed guidelines to review, prescribe or stop medications, make changes where applicable and discuss these directly with the patients.
Promote and implement reduction in practice spending, while maintaining patient safety.
You will be responsible for advancing patient care, applying clinical decision-making based on each individual and delivering collaborative working, as a unit, with both the staff situated at the surgery and external the practice teams alike.
Full support of both practice and locally executed policies and procedures will be required while providing guidance and direction within the practice where applicable.
You will be required to remain up to date on all aspects of medicine to follow new developments, updates to medication restrictions, cost-effective prescribing switches or other important information about prescription medications.
