We are seeking a dedicated and skilled Advanced Clinical Practitioner to join our healthcare team. Main duties of the job
Duties
* Conduct comprehensive assessments of patients to determine their health needs and develop appropriate care plans.
* Provide advanced clinical care, including diagnosis and management of acute and chronic conditions.
* Perform physical examinations, interpret diagnostic tests, and prescribe medications as necessary.
* Collaborate with physicians, nurses, and other healthcare professionals to deliver coordinated patient care.
* Educate patients and their families about health conditions, treatment options, and preventive measures.
* Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.
* Participate in quality improvement initiatives and contribute to the development of clinical guidelines.
* Stay current with advancements in medical practices and participate in ongoing professional development.
Job responsibilities
Key Responsibilities:
* Provide advanced clinical assessment, diagnosis, treatment, and evaluation of patients across a broad range of conditions and ages.
* Prescribe medications and treatments within scope of practice and according to local and national guidelines.
* Undertake face-to-face, telephone, and online consultations, including home visits when required.
* Work collaboratively with GPs, nurses, pharmacists, and the wider multidisciplinary team to ensure integrated patient care.
* Triage patients and manage same-day demand efficiently.
* Provide health education, disease prevention advice, and lifestyle support.
* Contribute to clinical governance, audits, service improvement, and protocols.
* Supervise and support junior staff and trainees where appropriate.
Person Specification
Qualifications
* A relevant degree in nursing or allied health profession with advanced clinical training.
* Registration with the appropriate regulatory body (e.g., NMC or HCPC).
* Significant clinical experience in a relevant healthcare setting.
* Strong assessment, diagnostic, and decision-making skills.
* Excellent interpersonal and communication skills, with the ability to build rapport with patients and colleagues.
* Ability to work independently as well as part of a team.
* Proficiency in using electronic health records and other relevant software applications.
* A commitment to continuous professional development and staying updated on best practices.
* Experience using EMIS Web (or other clinical systems).
* Familiarity with QOF and CQC standards.
