1. To ensure that the Trust has robust information to monitor liver surveillance pathways and community liver health pilot including; a. Identifying patients in at-risk groups b. Obtain key information from other Trust staff including secretaries, MDT Coordinators, Booking Clerks, etc. c. Obtaining key items of data from other systems and entering this key information into the liver surveillance data collection system d. Liaising with multiple agencies to book outreach events and ensuring adequate resources available e. Monitoring databases on several systems, across four provider trusts, entering data and escalating patients who are overdue surveillance. Building strong relations with the four provider trusts to retrieve the data required for submission and ensure that the data is collected for the Cancer Alliance in a timely fashion. f. Develop relationships with primary care colleagues, Drug and Alcohol Services and other multiple agencies to reach underserved communities 2. To collate the information required by NHSE for liver surveillance and the early diagnosis community liver health pilot a. Ensuring all data items required are captured in the liver surveillance data system accurately and concisely b. Ensuring data is collected and submitted in a timely fashion and is available for upload nationally within national deadlines. c. Summarise patient pathways for review and analysis d. Highlight potential blockages in the system e. Manage weekly huddles to determine where clinics/ clinical van will be visiting 3. To assist in the provision of both routine and ad-hoc information to the Trust Board, Directorate Managers, Clinicians, and Commissioners: a. Supplying information for inclusion in key reports b. Providing ad-hoc information requests pertaining to diagnostics c. Assisting in the development of information reports from the databases and systems across the four provider trusts 4. To ensure that data is robust and fit for purpose by: a. Ensuring all data is validated prior to its inclusion in any reports entry onto the database b. Liaising with relevant data quality leads in order to highlight any problem areas c. Providing both routine and ad-hoc validation checks on data held on trust systems 5. To ensure all procedures are fully documented and that all work undertaken complies with both Data Protection and Caldicott recommendations 6. Supervision and training of admin staff, providing support to administrative team.
