Job overview
We are looking to recruit a compassionate and dynamic Advanced Clinical Practitioner with good organisational and communication skills to join our friendly and supportive team at Ripley Medical Centre.
This is a great opportunity for an ACP with diverse skills to work as a primary care ACP, managing minor illness and common GP surgery presentations plus long term conditions alongside the wider clinical team.
You will need to have the ability to work independently within your professional scope of practice, to undertake patient assessment, diagnosis and clinical management within the General Practice environment, whilst working collaboratively within the practice team to meet the needs of our patients and supporting the delivery of policies and procedures.
The practice prioritises education and development so is looking for a self-motivated, enthusiastic individual with a desire to learn and support others.
Main duties of the job
1. Assess and examine patients, diagnose and develop appropriate clinical management plans.
2. Assessment may be face to face, or by telephone as part of triage, and will include patients presenting with undifferentiated problems.
3. Diagnose and manage patients presenting with both acute and chronic conditions.
4. Develop clinical management plans which may include medications and other interventions.
5. Review of patient's progress and re-evaluate clinical decisions as situations evolve.
6. Assess and manage patients' physiological and psychological needs including patients with complex needs, multiple diagnoses and polypharmacy.
7. Identify health priorities and make timely and appropriate interventions to support patients presenting in an emergency and/or with complex/urgent needs.
8. Identify patients at risk of developing a chronic illness and develop a plan for minimising the risks of progression.
9. Advise and educate patients, promoting understanding, self-management, compliance with a treatment plan, healthy lifestyle choices and sources of support.
10. Ensure all clinically generated administration is completed in a timely manner.
Working for our organisation
Ripley Medical Centre is run by the Derbyshire Community Health Services NHS Foundation Trust (DCHS).
We have a purpose-built surgery in the market town of Ripley, with a list size of approx 6,000 patients and free onsite parking.
The clinical team comprises 7 salaried GPs and up to 4 GP Registrars, an experienced ACP (in addition to this post), Practice Nurses and an HCA. The management team is formed by our Practice Manager, Deputy Manager and Clinical Lead, and we have a good team of reception and administrative staff to support with the day to day smooth running of the practice.

Experience
1. Clinical experience at senior level.
2. Management of minor illness, minor ailments, and injuries.
3. Clinical Leadership.
Qualifications
1. Professional registration with NMC or HCPC.
2. MSc in Advanced Clinical Practice (or equivalent).
Knowledge and Skills
1. Advanced clinical practice skills including Management of patients with long-term conditions.
2. Management of patients with complex needs and chronic diseases.
3. Practical understanding of primary care and community services.
4. Implementation of best practice.
