Job Description:
Working as a Social Prescriber Link Worker, you will play a pivotal role in connecting individuals with local community resources and services, facilitating access to support that addresses their social, emotional, and practical needs. This position is instrumental in enhancing overall well-being by fostering community engagement and empowering individuals to navigate and utilise available resources effectively.
Responsibilities include, but are not limited to:
1. Conducting thorough assessments to understand the social, emotional, and practical needs of individuals.
2. Engaging in personalised consultations to establish trust and gather essential information.
3. Identifying and connecting clients with diverse local community resources, services, and activities that align with their specific needs and goals.
4. Establishing and maintaining strong relationships with community organisations and service providers to ensure up-to-date knowledge of available resources.
5. Collaborating with clients to develop tailored support plans, setting realistic and achievable goals.
6. Providing ongoing guidance and encouragement to empower clients in achieving their objectives.
7. Cultivating relationships with local community groups, charities, and organisations to expand the network of available resources.
8. Organising and facilitating group activities, workshops, and events to promote social connections and community participation.
9. Maintaining accurate and up-to-date records of client interactions, progress, and outcomes.
10. Preparing regular reports for internal and external stakeholders to demonstrate the impact of social prescribing efforts.
Requirements:
1. Relevant qualification relating to Social Prescribing.
2. Previous experience in a similar role, preferably within a healthcare or community organisation.
3. Strong interpersonal and communication skills.
4. Empathy and the ability to build trust with diverse individuals and communities.
5. Knowledge of local community resources and services.
