· The post holder will be a member of the Complex Physical Health team (CPH) within the Community Adult Learning Disability Team.
· They will provide specific skilled support to aid the CPH nursing team in the assessment and management of adults with a learning disability and Complex Physical health needs across the borough of Bexley.
· They will help support with training to service providers and carers across the borough of Bexley.
· The post holder will be expected to facilitate group work with patients and carers.
· They will share a caseload with the Complex Physical Health nurses and will be expected to work unsupervised at times.
The Specialist Support Worker will work as part of the Complex Physical Health Nursing Team with a shared caseload of people with Learning Disabilities and Complex physical health in the community who require support to maintain and improve their physical health and wellbeing.
The post holder will have nursing supervision, and will work alongside Occupational Therapy, Speech and language therapy,, Physiotherapy, Mental Health in LD Nursing team and Psychology,

• Under the guidance and supervision of the Senior Community Learning Disability Nurse, the post holder will be expected to hold a small caseload of patients with learning disabilities and complex physical health needs, taking responsibility for implementing and reinforcing interventions.
• To report back patient progress or problems to the qualified clinician.
• To work with patients individually or in a group, in mainly community or clinic settings.
• To engage and participate in the assessment of patients in the community or clinics including:• Information gathering through liaison with patients, carers and the MDT.
• Monitoring an individual’s physical health and well-being.
• Contributing to clinical and non-clinical reviews, this may include obtaining vital signs (blood pressure, temperature, pulse, heart rate, respiration rate) for which, training will be provided.
• Liaise with community or clinics to support care delivery.
• Attend multi-disciplinary meetings to support an individual’s care and management.
• To attend local team meetings including attending referrals meetings as and when required.
• To support the Complex Physical Health nursing team in the delivery of learning disability training to community staff and carers.
• Be computer literate and to be able to use the trusts record keeping system and use documents such as Microsoft Word, Excel, Teams etc.
• To provide specialist support to patients with complex physical health and learning disability related appointments when necessary.
• Have a good understanding and awareness of the rights of patients and carers and assisting them in exercising those rights where appropriate.
