Responsibilities include, but are not limited to:
* Conducting thorough assessments of individuals in need of care, considering their physical, emotional, and social needs.
* Collaborating with service users, families, and other professionals to develop comprehensive care plans that promote independence and well-being.
* Advocating for service users' rights and choices in accordance with the Care Act.
* Providing emotional support and practical assistance to individuals and their families, ensuring their voices are heard and respected.
* Implementing safeguarding procedures to protect vulnerable individuals from abuse or neglect.
* Working closely with other agencies to investigate and address safeguarding concerns.
* Ensuring that care plans are implemented effectively, monitoring and adjusting as needed.
* Maintaining accurate and up-to-date records of assessments, care plans, and interventions.
Requirements:
* Degree in Social Work and registration with Social Work England.
* In-depth knowledge of the Care Act 2014 and relevant legislation.
* Previous experience working as a social worker in adult services.
* Strong assessment and analytical skills.
* Excellent communication and interpersonal skills.
