Job summary We have an exciting opportunity for a highly motivated and well organised Community Nurse to manage the Community Nursing Team in Bodmin and three harbours primary care network. You will be responsible for ensuring high quality evidence base, place based care. The team is geographically clustered around the Bodmin and Par areas and the team work from a base in Bodmin Health Centre. You will need to have experience in Community Nursing and leading a team. You will be part of the wider North Cornwall leadership team which includes Community Nursing, Home First, Community Rehab team, Specialist Palliative care and Integrated Respiratory team. Responsible for the delivery and maintenance of high quality evidence based care, focussed on meeting the needs and choices of patients and carers. To foster positive relationships with local commissioning boards and community and voluntary. Responsible for creating an effective, integrated multi-disciplinary approach within the locality.
