Main duties of the job As a Paramedic within our PCN, you will play a vital role in supporting our practices and patients, ensuring they receive the appropriate care promptly and effectively. You will work closely with our MDT and other healthcare professionals to provide holistic care for our community Key Responsibilities: Delivering high-quality care to our patient population compliant with the DES Work collaboratively as part of a multi-disciplinary team with GPs, nurses, and other primary care professionals within the PCN. Contribute to the delivery of the PCN model, ensuring it reflects local priorities, addresses health inequalities, and supports population health management. Provide direct clinical care to patients, including assessment, treatment, and referral as appropriate. Collaborate with multidisciplinary teams to enhance service delivery and patient outcomes.
