We are looking for an enthusiastic, hard-working, and motivated person to join our Stroke Therapy Team as a Rehabilitation Assistant. You will support our multidisciplinary team to provide an efficient and high standard service to our stroke patients across the New Forest. The key elements to this role will involve working with patients individually or as a group to support them with their rehab exercises and functional tasks, supporting our therapists within patient sessions and with administrative tasks, completing routine assessments and treatment plans.
This is role is a fixed term, part time (22.5 hrs over 3 days) role to cover maternity leave, previous stroke/neuro experience is desirable, but not essential. The successful candidate will complete stroke specific competencies and be provided with training. They will also participate in our IST programme, supervision structure, service development and our weekend rota supporting our 7 day a week ESD service. Applicants must have full access to a vehicle for work and possession of a full driving license is essential.
Our service uniquely covers 3 areas, including Early Supported Stroke Discharge (ESD), a 16-bed inpatient unit (8 stroke and 8 complex medical rehabilitation beds) and the Stroke Review Service for the whole New Forest and surrounding areas. The role is Community focussed but based within Lymington New Forest Hospital.
Main duties, tasks & skills required
To work with patients undertaking therapy/rehabilitation programmes to support and encourage them to achieve their personalised goals, optimum level of independence and quality of life, as directed by the team of therapists/nurses. Working with the individual patient or in groups.
To implement specific treatment programmes as specified by team therapists and nurses to stroke patients on the ward and in the community. This follows direct handover from the delegating therapist(s) or reading and implementing written treatment plans.
To administer routine assessments as directed by the Therapist/Nurses and in-line with individual competencies.
To undertake a range of clinical and non-clinical tasks including administrative tasks.
To issue, demonstrate, adjust and ensure safe use of personal aids/equipment for patients as prescribed and directed by therapists to support functional ability.
To employ practical skills e.g. occupation skills - cooking, household tasks, personal care, exercise/mobility programmes as a routine part of patient treatment programmes.
To be able to confidently communicate and liaise with patients, relatives, and other health professionals.
To provide feedback to professionals on patients' progress, changes in functional ability, medical, social, emotional etc.
To maintain accurate, comprehensive and up-to-date documentation- electronic and paper, in line with Trust policy and departmental requirements including collection of audit data.

Qualifications
Essential
NVQ III Health Care, or recognised appropriate equivalent qualification or competency based equivalent level of knowledge
Current UK driving licence and have access to transport with appropriate business insurance.

Previous experience as health care or rehabilitation assistant

Experience of working with people with health care needs.
Basic understanding of clinical conditions and disability.
Basic IT skills.
Exceptional interpersonal skills with the ability to communicate effectively with patients, carers and relatives remaining sensitive and empathetic.
Excellent organisational skills, ability to manage own time and plan timed activities for staff and patients.
Ability to recognise and manage challenging situations in a calm and professional manner.
Able to take instruction and direction and work effectively as part of a team.
Ability to record and retrieve information on charts/paper and electronic patient records.
High standards of written communication skills with the ability to use email and internet.
Evidence of a good standard of Literacy / English language skills.
Desirable
Experience of working with Stroke survivors or people with a neurological condition.
Knowledge of assisting Occupational Therapy, Physiotherapy and/or Nursing professionals.
To be flexible in working between an inpatient and community setting.
