Job Description
Qualified Social Worker, Assistant Team Manager, Swindon Borough Council, Adults Hospital Discharge Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.
The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.
Responsibilities will include:
1. Promote a culture of continuous improvement, through ownership of performance and maintaining quality assurance systems for the services.
2. Deliver the vision of the new Hospital Discharge Policy and support the team to provide operational cover 7 days a week, offering flexible working patterns over 7 days.
3. Work closely with acute and community health colleagues to ensure safe and effective discharges from the acute as well as working to prevent hospital admissions.
4. Liaise with other professionals.
5. 36 hours per week (flexible by negotiation).
6. 2 Month Temporary Contract.
7. Start date subject to DBS and reference clearance.

The successful candidate will need to hold a DipSW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver.
