The Newcastle Learning Disability Team is excited to offer an opportunity for a Band 6 Community Practitioner to work in our positive behaviour support pathway.
We are a multidisciplinary community team working across Newcastle with individuals with complex needs. The successful post holder will demonstrate a strong commitment to high-quality patient care, good organisational skills, and the ability to prioritise workload. Key components of the role will be to ensure the service offers a safe, caring, responsive, and well-led service.
The role includes liaising with agencies such as social care, service providers, inpatient services, and safeguarding. We pride ourselves on having a broad range of knowledge in these areas and the ability to manage associated risks.

Main Duties of the Job
1. Be responsible for assessing and managing a caseload of patients with substantial and complex needs.
2. Provide specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies, and the public.
3. Deliver high-quality assessment and care while offering support, advice, and supervision to the wider team.
4. Work collaboratively with patients to develop care plans, recovery, and wellbeing plans, ensuring regular reviews to monitor progress and adapt care plans as necessary.
5. Consider carer needs and ensure carer and family involvement in care planning where appropriate.
6. Deliver evidence-based clinical interventions.
7. Work as a member of the Multi-disciplinary Team to ensure high-quality individualised care, promoting recovery, wellbeing, and maximising independence.
8. Coordinate treatment planning, interventions, clinical reviews, and discharge planning.
9. Provide clinical supervision to staff as delegated by the Pathway Manager and Clinical Leads.
10. Provide compassionate care based on empathy, kindness, respect, and dignity.
11. Incorporate a patient-centred philosophy into nursing practice and collaborative care.

Person Specification
Knowledge and Experience
* 12 to 24 months of band 5 qualified nurse experience, dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
Education and Qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
* Experience of working in a community team.
* Post-registration qualifications in clinical interventions.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex health needs.
* Demonstrate an understanding of effective and appropriate treatment options, including a range of evidence-based clinical approaches.
* Clinical leadership.
