The successful applicant will need to be flexible, have relevant experience, possess excellent communication skills, and actively contribute to team working. You will be supported by the Senior Staff and the Team Manager.
We are seeking full-time applicants; however, part-time hours will also be considered. A valid driving license and access to a vehicle for work purposes are required.
This position is open only to applicants from the Hampshire & Isle of Wight NHS system, which includes: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, or Hampshire Hospitals FT.
Main duties of the job
To provide a high standard of clinical care, encompassing a holistic approach under the direction of a trained nurse, in patients' homes.
Working within a defined community nursing caseload for a group of GP Practices as part of the Integrated Community Care Team.
The post holder will have responsibility for maintaining safe practice by following relevant Trust policies according to the clinical setting. Holistic nursing assessments and care plans are recorded in line with Trust and NMC record-keeping standards, ensuring clinical care meets acceptable standards of patient safety and quality outcomes.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for health protection and promotion programmes that improve and reduce inequalities.
