About the Role An exciting opportunity has arisen for a fixed term Commissioner role for 12 months within the Out of Hospital Team, Older People’s Commissioning at Warwickshire County Council. This role is diverse, and you will gain experience working of working closely with partners in health including South Warwickshire University Trust and the Integrated Care Board. You will lead on some innovative commissioning areas including; Lead a contract that supports people to get home from hospital known as ‘the Hospital to Home service’. Lead the continued development and review of the Hospital Social Prescribing service ensuring that this service is sustainable moving forward. Work in partnership with the provider Healthwatch to ensure this service delivers its requirements. Oversee the Co-production framework ensuring that the funding is targeted and used appropriately to so that people that use services, and their carers are able to influence and shape future services and reviews. Lead Direct Payments commissioning linking with operational colleagues to support and maximise uptake of Direct Payments and undertake a strategic review of Direct Payments. Support work linked to the mobilisation of the Community Integrator Contract with SWFT as Lead Provider.
