Job summary Advert may close early if sufficient volume of application is reached The District Nursing sister will be responsible for providing strong clinical leadership and robust caseload management within an allocated PCN for a defined caseload of patients.The District Nursing Sister, as an autonomous specialist graduate level practitioner as recorded by the NMC, will have responsibility for patient assessment, care planning, implementation and evaluation of high quality professional care for patients living in their own or residential care homes.Post holders will be expected to work 7 day working patterns subject to the needs of the 24 hour District Nursing Service. Post holders may be required to support in other District Nursing teams / PCN's or localities at times of surge or escalation to maintain patient safety within the service.The post holder will support the Operational matrons in the management of staff including undertaking supervision /1:1's, deployment and teaching of staff and patients within a Primary Care Network (PCN)as well as supporting the ongoing development of the District Nursing service working towards the wider aims and objectives of both the Healthy Lives CG and Trust.The post holder will be responsible for developing and promoting a positive team culture that enables individuals to openly question, reflect and learn from their practice to ensure safety and quality are the forefront of all patient care and interventions. Main duties of the job We are inviting applications for a highly motivated and dynamic District Nursing Sister to join North Tees & Hartlepool Community Nursing Service. This service will provide care to patients within a range of settings including patients own homes and residential care homes. The post holder MUST hold the District Nursing Degree Qualification and a V300 non medical prescribing qualification. You will be required to work autonomously to provide a whole person approach to the care and management for patients within this setting. The post involves liaising with a variety of health and social care providers and other agencies to ensure the provision of high quality care management to this group of patients and their carers. We require a first level nurse who has excellent communication skills and who is confident working across professional boundaries. You will need to be flexible, adaptable & a good team player with the ability to work independently within a community setting.
