Are you an experienced Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 6 nurse to support the changing face of community health care. As part of the Urgent Community Response initiative, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect by completing clinical triage at the point of referral.
You will be working as part of the Rapid Response team predominantly on triage and assessment of referrals into the service, mostly via our partnership work with EEAST Ambulance Service and our UCR 'professionals line'. You will work from Queensborough House, Dunstable and be based with our Single Point of Access Team to provide a nurse clinician to support day to day as as senior presence and support nursing queries coming in to the team from varying referral routes.
Excellent communication, time management skills and sound clinical knowledge, is essential. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

Youmusthave a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-20:00. Please see JD and PS attached for more details.
• Provide high quality complex clinical triage using knowledge, skills, critical thinking and clinical assessment utilising criteria and clinical pathways.
• Provide healthcare advice supported by clinical pathways and criteria, ensuring timely onward referral to the appropriate Primary Care at Home team
• Be available to staff a 7 day a week service model between 08:00 – 20:00
