Main duties of the job As a band 6 cardiac specialist nurse within the team you will be required to assess, plan, deliver and evaluate patient centred care including medication optimisation, symptom management, deliver cardiac rehabilitation programmes, and education to aid improved quality of life. You will be working in partnership with patients, carers and other health and social care and voluntary agencies to promote health and well-being. You will be required to work multi professionally as part of the Cardiac Service, providing care for individuals requiring management of their heart failure and cardiac rehabilitation. You will be required to support patients within their own homes, community clinics, cardiac venues, and virtual appointments.
