An exciting opportunity has arisen for a Band 5 Pharmacy Technician in the Community Mental Health Team in the West. The job purpose is to provide a medicines management service to support the CMHT staff and patients. Responsibilities include establishing patient’s medication history, identifying discrepancies, addressing potential pharmaceutical issues, and collaborating with multi-disciplinary teams to resolve them. You will work with patients to optimise their medication such as clozapine, lithium, ADHD, and depot antipsychotics.
Main duties of the job
* Oversee clozapine dispensing in the dispensary and address problems.
* Manage depots.
* Conduct depot stock checks in CMHTs.
* Ensure medication stock facilities are in line with BCU standards.
* Participate in clozapine clinics.
* Perform formulary switches via EMIS.
* Check lithium bloods and refer to pharmacist where appropriate.
* Verify annual monitoring bloods and other physical health monitoring in relation to antipsychotics, lithium, ADHD medications, etc.
* Streamline dispensing for mental health in the dispensary.
* Ensure only RED drugs are being dispensed long-term.
* Transfer amber shared care drugs to GP once the patient is stable.
* Provide patient medication counselling (e.g., clozapine, lithium, antidepressants) to improve medication adherence.
* Run EMIs searches.
* Ensure Valproate ‘prevent’ compliance.
* Communicate prescribing of RED drugs with GPs.
* Conduct medication reconciliation for patients discharged from psychiatric units or transferred from psychiatric liaison or HTT.
