We are looking for a motivated, enthusiastic, and dynamic individual who will be responsible for a managed caseload working closely within the paediatric and adult congenital nurse team and who can ensure the consistent delivery of high-quality patient centred care to the children, young people and their families within cardiology and cardiac surgery. A good understanding of current issues in healthcare both locally and nationally, involvement in improving patient safety, risk and waste reduction and the development of others and general support for peers, junior and senior colleagues are expected. Main duties of the job The post holder will co-ordinate the delivery of care for teenagers and young adults living with congenital cardiac disease aged 13-25 years, this will include facilitating and supporting transition pathways for those moving from paediatric into adult care and patients presenting with congenital heart disease between those ages. The postholder will manage a caseload of patients living with congenital heart disease. This will include supporting young adult patients through transition clinics, outpatient attendances, inpatient admission and intervention/surgical procedures as needed. They will be required to deliver holistic specialist nursing care encompassing bio-psych-social models. They will have the flexibility to review patients in the clinics and on the wards. The postholder will be expected to liaise between all relevant members of the multidisciplinary teams in adult and paediatric congenital cardiology services, primary care and other community-based services, and services close to the patients home which may be located out of the immediate locality. The postholder will be expected to work independently according to a plan agreed with service heads and within the overall framework of the services policies and procedures. The successful candidate will be self-motivated, able to work autonomously and have excellent communication and organisational skills.
