Job summary The Community Nursing Service within North East Lincolnshire provide nursing care and support to patients, families and carers in a variety of community settings supporting patients to remain in their own homes, preventing hospital admission and facilitation of early hospital discharges and managing end of life care. The Community Nursing Service works closely and in partnership with Primary Care Networks, Secondary Care, Social Care, Specialist Nurses and other healthcare staff to provide a quality service personalised to meet individual needs. The service supports and assists patients to manage ill health and long term conditions including elements of disability, thus allowing them to have the best possible quality of life, while maintaining dignity and respect. Main duties of the job Job title: Nurse Associate Team: Community Nursing Apollo Location: Cleethorpes Primary Care Centre 35 hours a week, Permanent Salary Range 4 £24,645 - £27,202 pro rata per annum You will work under the direction of the shift/team leader/manager in supporting other Health/Social Care Professionals and the delivery of Health and Social Care interventions to Service Users. You will be expected to independently manage and self-manage a defined caseload of less complex cases, evidencing problem solving and reasoning skills in response to unpredictable patterns of work. You will complete assessments and deliver support for the functional/clinical needs of patients in the community including chronic and palliative conditions. You will be responsible for developing and implement care/support plans under direction of the appropriate professional which may include clinical, social care and/or other practical activities with minimal guidance. You will monitor and report any unexpected changes in condition of the individual to the appropriate professional in a timely manner. You will manage care of individuals and carers where the support/care plan has been put in place and may need close monitoring, frequent review or where interventions to minimise and manage risk may be required. You will be required to utilise clinical skills including venepuncture and wound management with an on-going responsibility to maintain and improve clinical skills and knowledge as service needs demand.
