The Locality Social Work Teams cover both East and West of Middlesbrough working with Adults from 18 years old. The service is very keen to promote flexible working for all staff. Do you have the passion to support people with diverse needs including (but not limited to) autism, physical disabilities, learning difficulties and educational needs, people with acquired brain injuries and people who experience addiction and significant trauma? If so this job could be for you The role involves working directly using a person centred approach promoting independence and discussing how to support individuals to meet their goals and aspirations. You will undertake strengths based assessments under the Care Act 2014 and develop plans to meet the desired outcomes of both service users and carers alike.
