Short Term Care Centre is a 24 bedded in-patient unit. The post holder will support a multi-disciplinary team addressing the recovery needs of service users in order to prepare them for transition to a range of community settings.
Provide clinical leadership and high standards of clinical care to maintain and develop the service within a local and regional context.
Deputise and support the Unit Manager in aspects of service delivery, quality and performance.
To ensure adequate, competent staffing levels and provide leadership and role modelling for staff.
To provide assurance in areas of performance in aspects of finance, workforce, safety and quality.
To manage and provide timely submissions and responses to weekly, monthly reporting as requested by Head or Deputy Head of Service.
Implement quality improvement initiatives in line with best practice frameworks and standards.
To participate in relevant forums and meetings commensurate with role.
All LW staff are expected to be able and willing to work across a 7 day service.
Main duties of the job
1. Provide clinical leadership and high standards of clinical care to maintain and develop the service within a local and regional context.
2. Deputise and support the Unit Manager in aspects of service delivery, quality and performance.
3. Provide high standards of care to service users.
4. To provide clinical leadership, experience and act as a decision maker within the staff team.
5. To line manage a group of staff to include undertaking caseload management.
6. To ensure adequate staffing levels and undertake ongoing monitoring of safer staffing levels on shifts and at times of acuity and increased risk.
7. To have knowledge of areas of performance in aspects of finance, workforce, safety and quality and contribute to performance monitoring reviews and processes.
8. To have knowledge of and contribute to elements of timely submissions and responses to weekly, monthly and quarterly data required in line with commissioning contracting requirements.
9. Implement quality improvement initiatives in line with Commissioning, governance and best practice frameworks and standards.
10. To liaise with regional Commissioners and Regional Case Managers and prioritise bed management co-ordination for secure pathway.
11. To participate in relevant forums and meetings.

Key Tasks and Responsibilities of the Post:
1. Deputising for the Manager in a variety of meetings and forums and ability to contribute in a meaningful way or as part of personal development.
2. To have an understanding of the organisation performance requirements and monthly data submissions and ability to contribute to tasks delegated by the Unit Manager.
3. To act as professional leader in response to clinical practice issues which are often complex and urgent.
4. Contribute to Human Resources issues such as providing line management supervision to an agreed number of clinical staff. This would incorporate managing sickness absence, staff appraisal and performance issues in line with the organisations policies and procedures.
5. Represent the team positively and proactively at relevant meetings or forums, which relate to clinical practice, acting as lead professional in specified areas agreed with Unit Manager.
6. Ensure that there is effective delegation of responsibilities to other team members utilising and developing their skills and abilities.
7. Contribute to the creative creation of the roster for the team to ensure staff covers across seven days and effective use of resources. Awareness and responsiveness to staffing levels on an ongoing basis in the best interests of patient and staff safety using dynamic risk assessment techniques.
8. To liaise between the unit and other services in the pathway and community teams in order to improve care pathway opportunities for service users.
9. Requirement to engage in auditing as part of ongoing service development, able to contribute to the collection of data and clinical information for reporting purposes and commissioning requirements as delegated by the Unit Manager.
10. To undertake SIRI, Complaints and HR investigations as per organisational requirements.
11. Support bed management for Short Term Care Centre. To include undertaking access assessments using relevant access criteria in line with regional guidance as delegated by the Unit Manager.
12. Deputise if required and implement actions generated and update Manager.
13. To maintain extensive knowledge of the patient flow through the unit and expected date of discharge and escalate any barriers to discharge.
14. Maintain communication with Case Managers and Commissioners.
15. Ensure whiteboards meetings or equivalent are held on a regular basis and participate when required.
16. Participate in the peer review process on an annual basis and membership of the Quality Network.
Professional / Clinical Leadership:
1. Provide clinical expertise on service user care in a supportive/mentoring and empowering role to the nursing team.
2. Facilitate the effective use and development of multi-disciplinary team meetings and the effective review of service user care using the CPA framework and My Shared Pathway.
3. Undertake reviewing and audit of clinical records as part of maintaining standards assessment or record keeping of the Care Programme Approach.
4. Contribute to case conferences and reviews, supporting service users and carers to engage with services and contribute to their care.
5. Give clear direction to junior staff around provision of care and standards of care.
6. Awareness of the requirement to ensure that the unit meets the requirements as outlined by the CQC to deliver safe and effective care.
7. Demonstrate the ability to manage and lead a team’s development and improvements in their clinical practice.
8. Monitor standards for clinical practice and patient care and ensure teams have the right skills to deliver a high quality service.
9. To support and ensure the implementation of clinical supervision for staff within areas of responsibility.

1. Demonstrate accountability of involvement with user/carers in practice within their immediate practice setting.

4. Frequently critique the nursing interventions of the team against latest evidence to assess where improvements could be made.

3. Ensure the Head of Service/Deputy Head/Matron and Commissioners are informed in a timely manner of any major issues on the units if deputising for the Unit Manager.
4. Actively participate in a range of meetings ensuring information is shared and cascaded.
5. Act as a role model and support communication between team members. Encourage and facilitate the opportunity for staff to give each other constructive timely feedback in the best interest of team communication to promote positive teamwork and mutual respect.
6. Maintain up to date service user records on SystmOne in a timely and accurate manner in line with information governance standards.

Education and Development:
1. Ensure staff are competent to complete delegated duties.
2. Ensure student placements are of a high quality and services provide a positive learning environment, supporting Unit managers and ensuring that there are sufficient trained numbers of practice supervisors and assessors.
3. Provide support to staff that are practice supervisors and assessors and support the development of newly registered nurses with preceptorship programmes.
4. Work in line with guidance of relevant policies and guidance and maintaining ongoing knowledge.
5. Support competency development for all grades of nursing and support staff through line management and appraisal process.
6. Demonstrate sound knowledge of clinical governance and risk management.
7. Ensure that all new staff complete induction training and mandatory training as required.
8. Organise in-house training where appropriate.
9. Ensure clinical environment is suitable for students/trainees working within the service area and that they receive appropriate supervision.
10. Represent the service by participating in presentations at forums both internal and external to the organisation as requested.
11. Participate in regular supervision, clinical and line management, engage in reflective practice and be committed to continued development.
Responsibility for financial and/or physical resources:
1. Support the manager in effectively delivering care to the patients within budgetary constraints.
2. Attend the performance meetings and engage with budget reviews in the absence of the manager or as ongoing development opportunities.
3. Be an authorised signatory for staff payments and minor budgetary purchases in the absence of the Team Manager.
4. Responsible for care of own and all workplace equipment, in line with Health and Safety/infection control guidelines.
5. In conjunction with the Manager, ensure all action plans are put in place following infection control or estates assessments.
6. Where appropriate handle or process cash or patient valuables in line with local policies.
7. To immediately report any faulty equipment.
8. Have a role as Fire Marshall in ensuring the safety of the building and to ensure there are sufficient team members with this training to have a fire marshall available.
9. Report accidents, incidents and near misses, implementing corrective action where necessary and completing Managers report in a timely way.
Clinical Effectiveness:
1. Ensure the Unit undertake their relevant patient satisfaction surveys at agreed intervals also taking into account the views of patients and carers in liaison with the Patients Advocacy Services, Complaints and Litigation department and Service User Experience Manager.
2. Ensure that any complaints and significant events are investigated in full (using RCA), preparing detailed reports with action plans in liaison with Head of Service or Deputy Head of Service. Implement action plan and review trends.
3. To undertake change processes in the best interest of patient safety, service development improvements using QI methodology or other effective tools.
4. To contribute to the development, implementation and evaluation of a range of audits within the service area, e.g. health and safety, infection control, specific clinical audits, NICE and NPSA.
5. Ensure processes are in place to promote co-production for users and carer and service re-design initiatives.
Professional and personal development:
1. Participate in regular supervision, clinical and line management, engage in reflective practice as per policy and professional requirements and be committed to continued development.
