1. The post holder will, under appropriate supervision provide first point of contact for patients presenting with undifferentiated, undiagnosed problems, utilising history taking, physical examination, problem solving and clinical decision making skills to establish a working diagnosis and management plan working in conjunction with patients. 2. The post holder will undertake both morning and afternoon clinics within various GP sites within the PCN as well as home visits and administrative work. 3. Participate in treatment and preventative healthcare. 4. Provide direct clinical care to patients using established clinical guidelines. 5. Consult and refer patients to physicians, medical specialists and other healthcare professionals. 6. Provide one to one consultations to patients, both in General Practice and patients homes, taking medical histories, perform physical examinations, analyse the consultation, diagnose and explain the medical problems during the consultation. 7. Recommend and explain appropriate diagnostic tests and treatment. 8. Request and interpret results of laboratory investigations when necessary. 9. Perform specialized diagnostic physical exams and treatment procedures. 1. Provide patient education in preventative healthcare. 1. Conduct telephone consultations which may involve discussing the results and implications of laboratory investigations with patients as well as dealing with routine and emergency healthcare conditions.
