South Lakes Rapid Response are looking for Band 5 Nurse to join our team in the community.
We are seeking a hardworking, dynamic, proactive, individual to join our well established multi disciplinary team.
You will support the delivery of the urgent community response standard which aims to provide a 2-hour crisis response to patients who are acutely unwell within our local community to avoid unnecessary hospital admissions. As well as supporting the care and management of patients on the Virtual Wards and Rapid Response caseload.
This can be a challenging role, every day is different but exciting and rewarding.
Main duties of the job
The role requires fantastic communication skills, creative thinking and working proactively and collaboratively with others to ensure a high standard of care is delivered.
The role requires undertaking holistic assessments sometimes in complex and challenging situations to develop and deliver robust programmes of care for patients and carers. This is an exciting time to work in the community. The community is advancing and developing all pathways to prevent un-necessary hospital admissions and deliver robust care plans in the home.
You must be able to use your own initiative, work under pressure, keep to deadlines and have excellent nursing, organisational, interpersonal and communication skills. You should be flexible and able to work as part of a team.
We will provide a programme of induction and you will be appraised on a yearly basis with a view to developing knowledge and skills where appropriate.

About us
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
