Role Overview
The post holder needs to work with a delegated caseload from a team of Physiotherapists and Occupational Therapists and work without direct supervision to carry out assessment and treatment interventions. You will be part of a multi-disciplinary team of Physiotherapists, Occupational Therapists, and Rehabilitation Support workers and work with the wider Community-based multi-disciplinary team, including Doctors, Nurses, Advanced Clinical Practitioners, HCAs, Mental Health Nurses, and Community Hospitals.
You will support patients with rehabilitation and discharge planning.
Responsibilities
1. Assist the Physiotherapist and Occupational Therapist with rehabilitation plans.
2. Complete assessments within your scope of practice.
3. Cover rotation between the WEB-Community Rehabilitation Team and Exmouth - in-patient rehab Therapy.
Working Pattern
The position currently covers seven working days, 37.5 hours over five days, to support service delivery from 08:00 to 20:00.
