Job summary Fronks Road Surgery are seeking a highly motivated, enthusiastic and experienced nurse practitioner to provide support to our loyal practice team. This position is based on a part-time basis for 4 days per week but this is negotiable. The post-holder will be an experienced nurse, who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. Main duties of the job Assessing, diagnosing, planning, implementing and evaluating treatment and interventions, as well as caring for patients presenting with an undifferentiated diagnosis. Clinically examining and assessing patient needs from a physiological and psychological perspective, and planning clinical care accordingly. Assessing, diagnosing, planning, implementing and evaluating interventions/treatments for patients with complex needs. Proactively identifying, diagnosing and managing treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnosing and managing both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Prescribing and reviewing medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
