Job Title: Hospital Discharge Social Worker
Location: Hammersmith and Fulham
Rate: Up to £35 per hour
An amazing opportunity to join a hospital team in Hammersmith and Fulham council has become available. Based at Charing Cross Hospital, you will be an experienced Social Worker who is used to working in dynamic and fast-paced teams.
What will your responsibilities be?
You will have experience working in discharge teams, undertaking Care Act Assessments to a high standard, and working in multidisciplinary environments.

Qualifications and Experiences:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a full UK licence, have post-qualified experience, and hold a degree level or equivalent in Social Work.
