Advanced Nurse Practitioner - 22.5 hrs pw listing
To work on behalf of a federation of General Practices across Blackburn with Darwen in a variety of services to facilitate at scale working and drive quality improvement.
To act as an experienced, autonomous advanced practitioner who is able to work independently and in conjunction with other members of the clinical team.
The post holder will be an experienced caring nurse who can demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries.
They will work within a multidisciplinary team consisting of GPs, PAs, Clinical Pharmacists and the wider practice team to deliver excellent primary care services in and across practices and as part of integrated neighbourhood teams in BwD.
They will also have a key role in supporting the PCN to deliver the Network Contract DES specifications for care homes and enhanced access requirements.
The post holder will work collaboratively to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required.
In order to work at this level, NMC requirements for advanced practice must be met.
Main duties of the job
1. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis and/or with complex needs.
2. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
3. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
4. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
5. As an independent prescriber, prescribe safe, effective and appropriate medication as defined by current legislative framework and within scope of practice.
6. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
7. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
8. Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation.
9. Assess, identify and refer patients presenting with mental health needs.
10. Implement and participate in vaccination and immunisation programmes for both adults and children.
11. Provide expert nursing advice to other nurses working.
12. Participate in MDT virtual care home ward rounds, proactively identifying patients to be assessed.

Job responsibilities
1. Participate in MDT virtual care home ward rounds, proactively identifying patients to be assessed in conjunction with care home staff.
2. Work as part of the MDT to optimise patient care and refer to or liaise with other key services such as the dietetics team, Specialist care home nurse, district nursing teams and the palliative care team.
3. Attend any training courses which are identified as being key to current practice.
