The Department of Pediatrics at the University of Virginia (UVA) School of Medicine is seeking a dedicated RN Case Manager/Clinical Nurse to join our team. This role requires providing relationship-based care within a collaborative multidisciplinary team, ensuring optimal patient outcomes. Responsibilities:
Develop and implement individualized care plans, prioritizing the needs of patients and families. Establish and maintain effective relationships with patients, families, colleagues, and interdisciplinary team members to enhance the patient experience. Conduct clinical assessments, coordinate care, and implement interventions to promote healing and alleviate suffering. Demonstrate leadership by actively participating in shared governance and supporting the onboarding and precepting of new team members. Engage in lifelong learning through professional development activities, staying updated on best practices and evidence-based interventions. Adhere to clinical documentation guidelines, analyze outcomes data, and participate in performance improvement activities to ensure quality achievement. Embrace innovation by utilizing technology to support patient care and implementing evidence-based practices derived from nursing research.
Minimum Requirements:
1. Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire.
2. Experience: One year of relevant experience required.
3. Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required.

7. May be exposed to chemicals, blood/body fluids, and infectious diseases.
Anticipated Hiring Range:
$60,000 - $90,000 based on education and experience.
