Job Summary: To work as an autonomous practitioner in assessing, implementing and evaluating nursing care to children, young people and families served by the organisation. Work in partnership with other professionals and agencies to provide planned child and family focused care. To act in an advisory role to other professionals and staff from other agencies in respect of palliative care for children and families. Duties & Responsibilities: Responsibility for Staff Train and assess competence of Care Support Workers Acting as a role model within team, motivating staff and setting an appropriate example To contribute to the training, education and support of a range of both medical and nursing students on clinical placement with the Hospice. Co-ordinate shifts, off duty and sickness records in the absence of Team Leader. Responsibility for the childrens care Assess, plan, implement and evaluate plans of care for children in partnership with child, parents/ carers and other professionals Provide effective care, treatment and management within the hospice and/or community setting To ensure all documentation is complete and kept up to date in accordance with Hospice policy Liaise with healthcare and social care professionals across organisations to ensure an integrated package of care and support designed to meet the specific needs of the family, is delivered and co-coordinated. Ensure care delivered is delivered to predetermined standards Work in partnership to actively involve parents/carers and children in the implementation and evaluate of care plans. Provide information for children, young people and families, giving clear ongoing information as the childs illness progresses, ensuring that parents are empowered to maintain control and make informed decisions relating to the care of their child. Contribute to the protection of children at risk in accordance with the area safeguarding procedures. Responsibility for Resources Promote the safe and effective use of equipment and resources Inform and liaise with Team Leader in regard to resource needs, including skill mix Have a good knowledge of equipment available and process of appropriate equipment supply. Service responsibilities Attend meetings relevant to post and on behalf of the Team Leader or Organisation Implement changes to established practices and/or policies Participate in service development under direction the Head of Childrens Services. Participate in and conduct audits of service in partnership with Team Leader. Deputise for Team Leader in their absence Ensure practice is clinically effective and evidence based Responsible for own professional development Contribute to the development and provision of a safe, effective and efficient service
