Clinical Nurse Specialist (Palliative Care)

The role of the Seaton Hospice at Home Team is to provide specialist 24-hour support to patients with palliative care needs within the Seaton area. The role combines clinical skills with emotional and psychological support to provide a holistic, patient-centered service based on the individual needs of each patient. Our aim is to enable patients to remain in their preferred place of care and to have the best quality of life possible.

Job responsibilities
We are seeking a motivated nurse who would like to develop their knowledge, skills, and experience of working with patients with palliative care needs. You will have the opportunity to learn and gain experience through clinical reflective practice and continued professional development. You will be supported in this post by the Seaton Hospice at Home palliative care nurse specialists and the palliative care nurse team manager.
This is an exciting opportunity to be part of a professional, specialist palliative care team, who work alongside members of the wider multidisciplinary team of health and social care colleagues both in the community and the acute care sectors.
The Seaton Hospice at Home model integrates the available services within the community into a one-team approach to provide the best possible care for all patients who are diagnosed by their GP as palliative, regardless of diagnosis or complexity, guided by the individual needs and wishes of the patient and their loved ones. This service provides continuity of care throughout the patients' journey, enabling people to die in their preferred place, which is often their home.
The Seaton Hospice at Home Service is managed by the Royal Devon University Healthcare NHS Foundation Trust and is funded by the Seaton League of Friends. The service provides specialist palliative care 7 days a week, 365 days a year, with shifts covering 08.00-16.00, 10.00-18.00, and 12.00-20.00. After 20.00hrs a Clinical Nurse Specialist is on-call to respond to our patients who need advice or a visit overnight, thus providing specialist end-of-life care and carer support 24/7, avoiding unnecessary and unwanted hospital admissions.
Working Pattern: 15 hours per week. 08:00-16:00; 10:00-18:00; 12:00-20:00 with overnight on-call.
