About the Team
The team is based within East Surrey Hospital and is one of five of Surrey's acute hospital teams consisting of qualified and non-qualified staff. We carry out three broad functions: managing complex discharges, D2A visits and assessments, as well as our Duty Hub function. Senior members of the team will generally carry out specific roles within one of these but need to be familiar with all to assist where needed. This makes each day interesting and fulfilling with no two days being the same and great opportunities for learning and diverse practice.
In addition to working within the East Surrey acute hospital, the team also supports D2A discharges from the community rehab and step-down beds at Greathed Manor care home and Coppice Lea care home, as well as assessments in people's own homes for those under the D2A domiciliary care service. For this reason, team members need to be able to drive and have access to a car for work purposes.
We work in a hybrid capacity, with regular office/hospital-based work required normally 3 days per week depending on the needs of our service.
About the Role
As a Senior Social Worker, you will be expected to have strong experience in assessing the social care needs of adults referred to us and provide direct social work intervention to some of the most challenging and complex adults with needs and their carers. You will provide effective support to individuals using a strength-based approach to enable individuals to live as independently as possible with more choice and control.
As a Senior Social Worker, you will supervise and support staff, as well as work alongside the team managers to improve processes and shape the team. You will need to be flexible to work in a challenging environment. You will be well supported and will benefit from a structured induction programme.
You will regularly meet with adults to understand their care and needs, how they can be supported, and to provide the best outcomes for them. Your work will include but is not limited to:
* Care Act Section 9 and Carers assessments
* Partnership working with our acute and community Health partners, charity and voluntary organisations
* Mental capacity assessments and Best Interest Decisions
* Assisting to ensure duty hub work is completed and processed to support the hospital system
* Structured supervision of a small number of team members
This service runs from 9am to 5pm Monday to Friday and 9am to 5pm Saturday and Sunday. To manage a 7 days per week service, we have a roster of bank staff for weekends with anyone welcome to join as they like.
Full-time applicants will be contracted to a 36-hour week Monday to Friday. Some flexibility of hours is negotiable.

To be considered for shortlisting for this position, your application will clearly evidence the following:

* You will be motivated and flexible with high professional standards and sound knowledge and experience of current legislation and its application to practice in a social care setting
* An applied knowledge of The Care Act and The Mental Capacity Act and experience of undertaking safeguarding enquiries is essential alongside experience of working in complex situations using professional judgement and a range of interventions
* You will have experience of, and be able to provide, professional, reflective supervision, guidance and advice to less experienced social workers, including those completing ASYE and social work students
* Willingness to be based onsite each day when required
Surrey has both urban and rural areas, and social workers will be expected to have a valid driving licence to drive in the UK, access to a vehicle, and be willing to travel across a wide geographical area.
