What you can add: Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties. Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their well being. Signposting to appropriate care providers (including primary and tertiary care). Raising concerns to senior staff for support and management. Completing documentation and planning care for patients. Presenting cases to Multi-Disciplinary Team for discussion around care pathway. Liaison with colleagues nursing, occupational therapy, medics, psychology and admin. The role will involve, but not be limited to you performing the above. What you'll need: A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years Comply with NMC Professional Registration and keep up to date with current nursing practices *Full UK driving licence and own vehicle. *Depending on the Trust, you may require variable additional certification and skills.
