We our looking for a Nursing Associate to do that role as part ofa Primary Care Network (PCN) multidisciplinary team. The post holder will mainlyfocus on supporting patients with multimorbiditys like diabetes, chronickidney disease, serious mental illness and learning disabilities. They will beresponsible for ensuring the effective holistic provision of high-qualitynursing care to people in clinics at community events and in outreachenvironments. There is a mixture of clinical work and as well greatopportunities to work with patients in group learning sessions and engage withthe community at arranged events and group sessions.
Main duties of the job
This exciting role with see the Nursing Associatesactively contribute to a patients health and wellbeing through the delivery ofholistic care, a whole-person approach which considers, and equally values,physical, psychological, public health needs, social economic, spiritual andother factors in the assessing, planning and delivery of care. Providing allrelevant competencies (to be developed in line with the requirements of theservice) have been achieved, she/he/they will have delegated responsibility fora pre-agreed caseload under the supervision of a nominated clinician within thePCN multidisciplinary. The post holder will be integral to the team of clinicalpharmacists, specialist Mental Health HCA, care coordinators, care homepatients and a womens health hub, GPs, Nurses, Health and Wellbeing Coaches,Social prescribers and the wider local care partners like: Community services,hospitals, specialist for long term conditions along the patients pathway.

Ourpopulation have a higher prevalence of mixed mental and long-term conditionsbecause of the nature of our more deprived communities, particularly at anearlier age. To work within the Nursing Associate parameters ofpractice and deliver high quality, compassionate care to patients in their ownhomes/clinic environment.
To work with PCN colleagues to deliver care thatwill be focused on patients who have or are at risk of multimorbiditys byensuring they are monitored and have access to receiving the physical,psychological, and social assessments.
To work under the direction/delegation of anominated clinician, in accordance with an individual care plan, with a focuson promoting health and independence in a personalised care plan using the Whatmatters to me approach.
To carry out specific delegated clinical and caretasks and responsibilities to a high standard and competency, under thedirection and supervision of a registered nurse or other registered healthprofessional.
To proactively monitor and prioritise patientsappointments and visits within a delegated caseload, making autonomousdecisions within agreed competencies about patient care and seeking supportfrom colleagues as appropriate.
Supporting the team at events with clinicalknowledge and experience.
Supporting and managing group-based session tosupport the patients education and understanding of relevant medicalconditions like hypertension.
Work in an effective and organised mannerdemonstrating excellent time management and organisational skills toeffectively deliver person-centred care for an allocated group of individuals.
To contribute to the development of staff withinthe service through training and supervision.

Knowledge
* - Be able to use GP clinical Systems EMIS and SystemOne
* - Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation and how the role may contribute to service development
* - Knowledge of long term/health conditions (to appropriate level) e.g. Diabetes Heart failure Respiratory CKD SMI and Learning Disabilities.
