In conjunction with the Mental Health Lead Midwife work closely with Perinatal Mental Health Teams in care planning and assessment of women and families where there are mental health concerns. Work closely with community and charitable agencies and ensure that women and families are linked into additional support at the earliest opportunity. Engage in other public health initiatives and projects - especially in relation to improving outcomes for women and families with mental health concerns. Contribute to pre-birth planning, Multidisciplinary Team meetings and caseconferences as required, ensuring effective sharing of information. Attend the monthly internal Psychosocial meetings to support the mental health leads. Ensure that robust pre-birth/discharge planning and plan is in place with the supportof the safeguarding and mental health leads. Attend Postnatal Discharge planning meetings on the postnatal ward for any cases with social care involvement. Support staff on the inpatient wards with mental health cases. Ensure robust information sharing and care planning with primary care teams. Liaise appropriately with services to ensure safe and effective care is provided to women. Act as a care navigator to ensure timely access to appropriate pathways and robust information sharing and handover has been completed.
