The South Nottingham Integrated Community Stroke Service have an exciting opportunity for a dynamic and skilled band 7 Occupational therapist(OT) to join our well-established service. This post a 0.96wte = 36hrs permanent position.
As a Highly Specialist OT; you will work as an autonomous practitioner, working collaboratively within an integrated multi-disciplinary team. You will manage a complex caseload and provide expert assessment and treatment programmes for patients who require specialist stroke rehabilitation.
Our Integrated Community Stroke Service delivers rehabilitation into various community settings 8am-5pm, 5 days a week, including Bank Holidays. Weekend working may be required in the future.

Main duties of the job
This role requires day-to-day delivery of patient care in our Integrated Community Stroke Multidisciplinary Team of AHP's.
Main duties include utilising highly specialist clinical reasoning, critical thinking, reflection and analysis to support assessments, treatment and management of stroke survivors with complex presentations.
As a highly specialist OT you will work closely with the MDT, communicating effectively with clients/carers and their families and supervise, teach and develop other staff as required. Supporting our clients within the community to enable them to live their optimum life, being as independent with their health needs as possible following a stroke.
You will be expected to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable, evidencing high levels of personal and clinical credibility, being able to establish effective working relationships in a range of work settings.
You will be providing locality leadership and support to staff regarding the delivery of the specialist community stroke rehabilitation and ongoing case management as part of the specialist service.
You will play a key part in the development of stroke management within community services and ensuring commissioned targets & Key Performance Indicators are met.

Job responsibilities
We are looking for a Highly Specialist Occupational Therapist to join our dedicated Community Stroke Team across the South of Nottinghamshire.
This post will allow the successful applicant the opportunity to work closely within an integrated multi-disciplinary team, providing therapy to clients referred to the Community Stroke Rehabilitation services, including early support stroke rehabilitation. The multidisciplinary team includes Occupational Therapy, Speech and Language Therapy, Physiotherapy, Clinical Neuro-Psychology, Assistant Practitioners and Rehabilitation support workers. We also provide a post stroke review for clients at 6 and 12 months.
As a Community Stroke service, we aim to offer a high quality therapy led Multidisciplinary Team approach providing stroke rehabilitation in a patients home or care home, focused around achievement of agreed rehabilitation goals.
As a Highly Specialist Occupational therapist in the Community Stroke team you will be expected to:
1. Manage a caseload using highly specialist Occupational Therapy assessment for stroke clients who may have complex and diverse presentations, and plan appropriate management and treatment using specialist advanced clinical reasoning skills and a wide range of therapeutic skills.
2. To provide high quality holistic evidence-based care to patients/service users that is responsive, effective and efficient.
3. To facilitate and support timely discharge and prevent inappropriate admissions by supporting patients/service users to remain as independent as possible and maintain or improve their level of function and quality of life.
4. To assist in the development and implementation of appropriate pathways of care for the Stroke pathway.
5. To provide locality team leadership and deputise for the Team Lead when needed and provide support with the clinical development and management of staff.
6. To be pro-active in delivering responsive and anticipatory care to the wider community, promoting independent healthy lifestyles, reducing health inequalities, and tackling underlying determinations of health, to improve overall qualities of life in the local population.
7. To provide some operational leadership; ensuring commissioned targets & Key Performance Indicators are met.
8. To provide support and training to junior staff and students within the team to manage complex patients who require rehabilitation.
9. To support the organisation and delivery of in-service training and initiate informal training sessions when required to ensure high standards of clinical care.

The Community Stroke Team is split into three localities. The base of this role is within in the locality of Rushcliffe.
