The successful candidate will be part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT). Clinical leadership will be provided by the PCN Clinical Directors and Clinical Leads. The successful candidate will be based in a local cluster of General Practices as part of the PCN. We are looking for individuals who will lead clinical care home rounds & coordinate the work of healthcare professionals and non-clinical staff in the care of patients registered at GP practices within the wider PCN population. The role includes responsibility to work with the PCN Operational Manager and the practices to support and ensure compliance with the reporting requirements of the PCN Directed Enhanced Services (DES). Main duties of the job The role will involve working across organisational & professional boundaries to provide clinical care and leadership and co-ordinate the treatment, care, management, and empowerment of care home residents. The individual will take part in the weekly care home rounds to include - Lead weekly ward rounds in our aligned Care Homes - Weekly MDT's with our Learning Disability EHiCH colleagues in our LD Care Homes - Review medication The Enhanced Practice Nurse will work closely with the Care Coordinators, GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.
