An exciting opportunity has arisen within the Specialist service for bladder and bowel care. A job within Community care for patients with complex continence needs.
The role is based in Normanton H.C and covers care home, clinics, and patients' own homes around the community.
The role will consist of assessments on patients with continence needs, linked with multiple health conditions, and teaching ISC and catheterisation.
The successful candidate will work with the CNS team to deliver high-quality care for new and current patients.

Job responsibilities
1. Assess, plan, implement, and evaluate evidence-based nursing care plans.
2. Assist and support clinics for follow-up examinations and treatment adhering to clinical policies and protocols within the Urology investigations Unit.
3. Assist in the management and organisation of the clinical area.
4. Work collaboratively and co-operatively with the multi-disciplinary team.
5. Be computer literate with the clinic system.
6. Ensure compliance with policies, procedures, and clinical guidelines for self and others.
7. Continually monitor standards of care and contribute to improvements of care through benchmarking, audit, and research.
8. Participate in patient and public involvement activities.
9. Support people's equality, diversity, and rights.
10. Comply with the Data Protection Act and Caldicott recommendations.
11. Contribute to the multi-professional clinical governance/audit sessions.
12. Proactively support the clinical incident reporting system to improve practice and service delivery.
