Applications are welcome for the above Fixed Term, full time (40 hours per week) post based in REACT Hub, St Johns Hospital This 12 month Clinical Fellowship is based with the West Lothian Hospital at Home service, offering acute assessment and treatment in the patient’s own home as an alternative to hospital admission. The post-holder will gain a strong foundation in Medicine of the Elderly, developing skills and knowledge to assess and manage frailty and acute illness in the context of multi-morbidity, manage complex chronic disease, and work closely within the multidisciplinary team under close Consultant supervision. The Hospital at Home service is part of the Rapid Elderly Assessment and Care Team (REACT) at St John’s Hospital, NHS Lothian. REACT offers a single point of contact for assessments and interventions for frail older patients during an episode of acute deterioration. The team consists of geriatricians, nurse practitioners, clinical fellows, physiotherapists, occupational therapists, pharmacist, clinical support workers and administrative staff. We offer comprehensive geriatric assessment alongside acute interventions such as oxygen and intravenous therapies within the patient’s own home. This post also includes 1 weekly session of development time, during which the post-holder will be encouraged and supported to undertake an area of interest such as quality improvement, service development, teaching or subspecialty training. Applicants should have completed Foundation training and be able to drive a car to community home visits around West Lothian. Additional postgraduate experience and qualifications are desirable. Good communication skills and the ability to work flexibly and collaboratively within a multidisciplinary team are essential.
