This is a part-time, permanent post of 22.2 hours per week, with a shift pattern of 9am-5pm, 3 days a week.
Please note the salary is Pro Rata for part time hours.
Post holder will be a Registered Nurse and is required for West Dunbartonshire Community Childrens Home/ASL School Nursing Team (CCNT) as part of Specialist Childrens Services NHS GG&C.
The team delivers nursing care to children and young people 0-19 years in the community and in Kilpatrick (Additional Support for Learning) School with acute and complex healthcare needs.
Post holder will be expected to carry out Nurse Led Assessments within the School and also participate in delivering the immunisation programmes.
They will also be expected to carry out home visits and expected to work within the paediatric acute and community clinics and nurse led phlebotomy clinics at Acorn Centre.
They will be expected to carry out sleep work/melatonin reviews for the Disability Service.
The post holder must have the ability to work on own, making appropriate clinical judgements independently but also work as part of a multi-disciplinary team, promoting integrated nursing approaches.
The post holder will participate in assessment/review of children and young people including those with additional support needs.
The post holder will be responsible for development, implementation and evaluation of care plans, providing nursing care as necessary.
The post holder will promote the principles of self-care to children, young people, families and carers and empower them to access and use health resources in the community.
The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to lead/deputise in the absence of the Nurse Team Lead and enable the Community Nursing Team to continue to provide a quality service.
The post holder will be the clinical expert who will be accountable for case management and will ensure that service provision is effective, efficient and evidence based.
