To ensure the timely review of all people entitled to Funded Nursing Care who are registered to a BLMK GP, functioning as a central point for referral, assessment and support. This will contribute to the maintenance of the appropriate level of funding support, the accuracy of the CHC database and the commissioning of appropriate NHS funded packages of care to meet the identified care needs of individual patients deemed eligible for support. Co-ordination of the FNC assessment process and where necessary writing key elements of the patient care plan to reflect presenting health needs and reflecting this back to the CHC Assessor. Be able to work within a multidisciplinary team and family members to ensure the FNC assessment or review is undertaken appropriately. Support the CHC process by undertaking identified pieces of work. Interpreting national and regional guidance relating to the implementation of the National Framework and subsequent developments within all of the CHC service. In relation to the CHC process and applications monitor and track risks and issues establishing a proactive resolution and escalation processes in close liaison with the CHC clinical assessor. Provide coordination of and participate in relevant meetings, reporting attendance and providing information, advice and support where requested. Ensure that data collected in relation to FNC reviews is analysed, reported by the team as appropriate and monitor the processing of data and information, record information on the data system to reflect accurate and timely case notes are managed effectively. Provide relevant and timely specialist advice and guidance on functional and information matters relating to the FNC assessment and processes. Interpreting national and regional guidance relating to the implementation of the National Framework and subsequent developments within all of the CHC service. Exercising professional judgement as necessary when communicating information on behalf of the CHC service and application of the associated criteria. Offering continuing support and advice to family and carers in which will require a high level of interpersonal and communication skills. Working autonomously, making decisions and managing own diary whilst liaising with nursing homes to book appointments. This includes speaking with people and their families/next of kin but knowing when to seek guidance and advice. Contribute to policy development particularly with respect to FNC and CHC reviews. As required support the CHC clinical staff in the delivery of training. As required support staff you meet in relation to the discharge of your duties. Responsible for assisting in the development and maintenance of databases required for regular report in relation to the FNC reviews under the direct supervision of the CHC Assessor. Contributes to ensuring there are processes in place for reviewing FNC patients and share learning and outcomes. Shadow CHC Assessors to learn the skills related to completing DSTs Maintain continuous professional registration in accordance with the requirement of the appropriate regulatory body, ensuring renewal is on time. Participate in regular clinical and managerial supervision where required by your professional body.
