An opportunity has arisen for a suitably qualified and experienced Learning Disability Nurse or Mental Health Nurse to work as an AWP employee within the Community Learning Disability and Autism Diagnostic Service. This role is with the Specialist Learning Disability Health Community Team.
You will work as part of a multi-disciplinary team and manage a complex community caseload. You will supervise staff and students and be supported to develop your skills career with AWP.
The role involves CPA co-ordination, holding a caseload of clients who may have complex presentations, providing consultation and learning disability advice to mainstream services.
Main duties of the job
Working in a collaborative, pro-active and preventative way to reduce the need for admission to hospital and deliver interventions to promote recovery and wellness. You will work closely with other disciplines within the team, as well as those from other agencies, to to assess and monitor the health of clients and provide therapeutic interventions .
Working within Swindon area, you will work with service users in a variety of settings. This will involve travelling between visits and appointments.
You will have the ability to understand how physical and mental illness in people with Learning Disabilities may be different from a mainstream presentation. The ability to implement "reasonable adjustments" in the assessment, planning and delivery of physical and mental health care is an essential part of this role.
Liaison with mainstream services about what unique challenges someone with a Learning Disability might face in hospital is an important aspect of this role. You will support service users where possible to access mainstream services and will provide advice and consultation to these services.
