Job Overview An exciting and unique opportunity has arisen to be a part of the Mental Health Intensive Support Service in East Cheshire. This is a newly established Service that provides specialist assessment, treatment, interventions & support, to help people recover from complex mental health problems & to regain the skills & confidence to live successfully in the community. The team are incorporating an open dialogue approach when working with patients and families. This vacancy is fixed term for 12 months. Main Duties of the Job As a Senior Support Worker, you will be expected to support the MDT in providing intensive support to a small cohort of patients, providing high quality levels of care in order to assist these patients to live independent lives. You will be working alongside the Primary Worker to ensure effective and adaptive patient care with a strong focus on recovery, identifying relapse indicators and promoting independence. You will also be responsible for ensuring all patients engage in the agreed care plan and assist in encouraging inclusion in local community services, to promote daily living skills. You will be responsible alongside the MDT for active participation in training of Care Staff/third party organisations that will be in place in the community that support the patient, such as Supported Living staff or Care Agency staff. MHIST are proactive in pre-empting crisis and preventing admission into hospital, and therefore you must be able to demonstrate excellent skills in risk management, adaptive care planning and communication skills across a range of scenarios. Person Specification Qualifications Essential Criteria Recent relevant experience of working to support and integrate people with mental health issues towards recovery Experience of working within a Mental Health community setting, rehab setting or equivalent. NVQ Level 3 in Health & Social Care or Equivalent Knowledge and Expertise Essential Criteria Understanding of and commitment to principles of the Community Rehabilitation and Recovery Models Experience of producing and leading teaching and learning in presentational sessions/in groups.
