About the Role:
As a Care Coordinator, you will play a vital role in matching clients with suitable care providers, ensuring the highest quality of care is delivered.
Key Responsibilities:
* Matching clients with appropriate care providers based on their needs and preferences.
* Coordinating care schedules and ensuring optimal coverage.
* Building strong relationships with clients and care providers.
* Providing excellent customer service and support.
* Managing and updating client records and care plans.
* Handling administrative tasks, including scheduling, documentation, and reporting.
* Collaborating with the clinical team to ensure seamless care delivery.
* Empathy and compassion for elderly and vulnerable individuals.
Skills and Qualifications:
* Essential: SVQ Level 3 in Health and Social Care or equivalent.
* Proven experience in care coordination or a related field.
* Excellent organizational and time management skills.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Knowledge of care standards and regulations.
* Proficiency in using computer systems and software.
