This is an excellent opportunity for a social worker with strong experience in adult services to make a real impact in the lives of vulnerable adults within the community. As an Adults Social Worker, you will be responsible for assessing, supporting, and managing the care and needs of adults, including older people, those with physical disabilities, learning disabilities, and mental health conditions. You will work collaboratively with service users, their families, and other professionals to ensure that services are tailored to meet individual needs and promote independence. Key Responsibilities Undertake comprehensive assessments of service users’ needs, including carrying out the Care Act assessments for adults with eligible needs. Develop and review person-centred care and support plans, ensuring they reflect the needs, preferences, and aspirations of service users. Work closely with other professionals, including healthcare providers, GPs, and community teams, to ensure the delivery of high-quality care. Safeguard vulnerable adults through timely intervention and risk management, ensuring all safeguarding procedures are followed in line with legislation and best practice. Provide advice, guidance, and support to service users, families, and carers, ensuring they understand their rights and options. Promote independence and wellbeing by identifying and facilitating access to appropriate community resources, including reablement and preventative services. Ensure that all documentation and records are accurate and up-to-date, complying with regulatory and legal requirements. What We Are Looking For HCPC-registered Social Worker with current Social Work England registration. Proven experience in adult social care, including assessments and care planning for individuals with complex needs. Strong knowledge of The Care Act 2014, safeguarding adults procedures, and local and national adult social care policies. Ability to work autonomously, managing a caseload effectively and prioritising tasks in a busy environment. Excellent interpersonal and communication skills, with the ability to engage and build relationships with service users, their families, and multi-agency professionals.
