Main duties of the job Responsibilities Conduct comprehensive medication reviews and assessments to identify and resolve medication-related problems for patients. Collaborate with physicians and other healthcare professionals to develop and implement personalised medication therapy plans. Provide patient education on medication use, potential side effects, and drug interactions. Monitor and adjust medication therapy based on patient response and laboratory values. Conduct medication reconciliation and identify discrepancies to ensure accurate and up-to-date medication lists. Participate in multidisciplinary meetings to discuss patient care and optimise treatment outcomes. Provide education sessions for GPs, practice staff, other health professionals and patient groups. Stay up to date with current literature and guidelines to provide evidence-based pharmaceutical care.
