Job overview
An opportunity has arisen to join our Palliative care clinical nurse service. Two 37.5 hour posts have become available as a result of retirement and an internal promotion.
Both Band 7 CNS roles are located in Ysbyty Cwm Cynon and are based within our Palliative Care unit. The positions provide an outreach service to patients with palliative care needs across the community. While working as part of the 7 day roster, the service provides support to both the hospital and community area.
The specialist palliative care service is delivered within teams on multiple sites across Cwm Taf Morgannwg. An appreciation that on occasions, dependent on patient need there may be a need for post holder flexibility in providing support to cover the CNS role across other sites.
Main duties of the job
The position involves directly supporting and advising patients with palliative care needs in a hospital and community setting.
Multidisciplinary team working to lead education and to provide support and guidance to colleagues about the specialty is key. The core hours are Monday to Friday 9-5pm but with participation in a 7 day roster with the same working hours. The role will require giving advice to other health care professionals, patients and families, potentially visiting patients in the hospital and community setting at weekends.
The successful applicant would be responsible for managing a caseload of patients with specialist palliative care needs in collaboration with the wider MDT, participating in education, audit and providing leadership regarding the specialty.
You will need to be enthusiastic about the specialty, demonstrate experience of caring for patients with palliative care needs and be able to meet the travel requirements.
The Specialist Palliative Care Team in CTMUHB is a multi-professional team providing a clinical, advisory and educational service across CTMUHB.

* UK NMC Registered Nurse
* Qualification in palliative care at Degree/Diploma or equivalent skills, knowledge and experience, supplemented by specialist training, experience, short courses to Master’s Level equivalent
* Teaching and assessing qualification

* Significant post registration experience, with experience of caring for patients with palliative care needs
