As the Care Coordinator, you will play a pivotal role in organising the rotas for the care team, allocating referrals for new work and evaluate care plans. You will ensure that each client has reasonable cover and you will always have the customers’ needs at the heart of what you do. Your role will also involve: Ensure that the Care service is delivered to Service Users at the times and for the duration specified in the relevant individual agreement(s) and contract standards. Ensure that the staff allocated, and the staff attending, each client have the relevant skills, knowledge and resources related to the opportunities and needs of that client. Ensure that each staff member is introduced to the client before or at the beginning of the first visit by that staff member, and that the client always knows who to expect at each visit. Manage travel delays and staff shortages to mitigate to the fullest extent possible the impact on the client, managing and minimising their distress and insecurity in such situations. Assist in the development of the philosophy, goals and objectives for the Care practice. Assist in the assessment of the effectiveness of Care implementation and delivery. Ensure that the Care service is delivered in a safe and person-centred manner Take part in the on-call rota on a rotational basis What qualifications & experience are required? You will ideally hold at least Level 3 in Health & Social Care (Adults) or be experienced in a similar position; Full driving licence and access to a vehicle is desirable; Above all, you will be a positive, organised and efficient individual who is passionate about delivering high-quality home based care.
