We are seeking to recruit an experienced community support worker who has worked with people experiencing psychosis and other mental health conditions.
This role will include supporting individuals to improve their mental health by working with them, their families, members of the multidisciplinary team, and wider support agencies within the community.
Main duties of the job
1. Establish and maintain effective contact with service users, their relatives, and carers to ensure good communication.
2. Provide information and assistance to service users and their carers so that they are aware of their rights and entitlements from the team and wider health and social care community.
3. Work with service users and their carers to access identified services. Monitor mental health and be aware of fluctuations in mental health through observations and report changes to the team practitioners so that service users are directed to appropriate services and risk is managed.
4. Encourage social/physical activities as appropriate to the service needs.
5. Maintain up-to-date client records in a timely and accurate manner.

Person Specification
Experience
* Experience working with clients with a range of vulnerabilities, including mental health, substance misuse, and learning disabilities.
* Experience of working with clients who have social needs and sound working knowledge of various agencies, such as housing and benefits.
* Experience of managing the risk associated with the above vulnerabilities.
* Experience working within a community setting in either the statutory or voluntary sector.
* Experience of lone working with service users.
Qualifications/Training
* NVQ Level 3 in Health & Social Care or equivalent experience.
* Training to Level 2 in Safeguarding Children & Young People: roles and competencies for Health Care Staff.
