Norfolk Primary Care (NPC) isan established PrimaryCare provider ofclinical services. We are a key provider of Primary Careservices in theNorfolk & Waveney system working withthe local GP practices.
NPC has been sub-contracted toprovide a home visiting service to the Practices in the Norwich PCNs. This comprises of a team of clinicians working across the Norwich PCNs. Thestaff provides additional Primary Care support to that currently being provided by individual practices.
Supportedby a Service and Clinical Lead, the HVS team are responsible for working withall HVS participating Practices focusing on proactive support to patientsresiding at home (including care homes).
The aim of the HVS is to reduce the number of avoidable admissions into hospital and to delivermore consistent, enhanced services from Primary Care. It offers proactive health caresupport to housebound patients, alongside the general population of patientswho may require home visiting services.
Main duties of the job
This exciting role will provide a high-qualityservice to housebound patients on behalf of their registered GP Practice, andthe wider PCN.
You willbe an experienced clinician who will be acting within their professional boundariesand competencies. You will provide carefor the presenting patient from initialhistory taking, clinical assessment, diagnosis, treatment, referral (where required), evaluation, and follow up ofcare.
You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills,for the patients withinPrimary Care. The post holder will demonstrate critical thinking in theclinical decision-making process. They will work collaborativelywith the GP Practices to meet the needsof patients in supporting the delivery of policy and procedures and providing nurseleadership if / as required.
In order to work at this level,those registered with the NMC / HCPC,requirements for Advanced Practice must be met.
